Appointment Policy

New Patients must arrive 30 minutes prior to appointment with ID & Insurance cards, medication list, and their completed paperwork.

Existing Patients must arrive 15 minutes prior to their appointment with ID & Insurance cards, medication list.

If you are a patients POA and will need to complete any paperwork or sign anything on a patient’s behalf, you MUST provide or already have POA document on file.

We understand that there are times when you must miss an appointment due to an emergency. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed treatment.

Cancelled Appointments

Woodlake Podiatry, LLC requires 24-hour notice for cancelled appointments; otherwise it will be considered a missed appointment. Any missed appointments will be subject to a charge as outlined in our financial policy. This is not covered by your insurance and is due before your next appointment can be made.

Cancelled/Returned Checks

Any cancelled or returned checks will be subject to a $35 fine. This is required to be paid before any future appointments will be scheduled.

Nail Care Appointments

No nail care appointments will be available after 4:00 pm.
